New Patients

All new patients need to fill out a Registration Form and a Medical History Form. These can be obtained from reception and you will need to fill in a separate form for each new patient who wishes to register. To save you time, you can download the forms here and print them off to be filled in at home. You can then bring the completed forms with you when you come in to register.

When you first visit our practice, we will check your teeth and mouth; measurements of gum health will be made and your mouth will be screened for oral cancer and, if necessary, take x- rays. We will also take a detailed medical & dental history.

After listening to your concerns and discussing treatment options we will create a personalised treatment plan based on the information from this visit. At your next visit, we will clarify any questions you may have about the treatment plan and its outcomes before starting your treatment.

Children`s Dentistry

We go out of our way to make children's dentistry as enjoyable, fun and pain- free as possible. We believe in encouraging good habits early on and take a preventative approach to treatment. We like to see children as young as possible to encourage good brushing and diet. We are a family practice and enjoy treating whole families some of whom spread over four generations.

Exempt Patients

Exempt patients will be required to show their exempt documentation at the first appointment. Our practice staff will be able to assist with any enquiries you have regarding payment exemptions.
